We are seeking a highly organized Case Manager to join our team in Atlanta, GA. The successful candidate will be responsible for managing a caseload of clients, coordinating services, and ensuring that clients receive the support they need to achieve their goals.

Responsibilities:
  • Manage a caseload of clients and provide ongoing support
  • Coordinate services and resources to meet the needs of clients
  • Develop and implement individualized service plans
  • Monitor client progress and adjust service plans as needed
  • Collaborate with other service providers and community resources
  • Maintain accurate and up-to-date client records

Requirements:
