Who we are:

Since 1985 Brandywine Counseling & Community Services, Inc. (BCCS) has been a trusted provider of outstanding substance abuse and behavioral health treatment. BCCS is a community organization that prides itself on helping members of our community recover with dignity. We enhance the quality of life by creatively providing Education, Advocacy, Prevention, Early Intervention and Treatment Services. We promote hope and empowerment to persons and families touched by mental illness, substance abuse, HIV and multiple occurring diagnoses, and their related challenges.


As a TCM Care Manager with BCCS, you will:

  • Participate in assessment, evaluation, planning, linkages, monitoring, advocacy and treatment of assigned clients.
  • Provide services to clients who are dealing with substance abuse and/ or mental health issues.
  • This program is a cooperative effort with our team of TCM & PSS to provide assessment, coordination, and monitoring services to referred clients.
  • Responsible for completing a Well Being Care Management assessment on all clients, thereby, taking that informing and creating an Individualized Care Plan.
  • Must maintain 75% of all client contacts are face-to-face.
  • The Care Manager will aid the linkage of services needed by the client.
  • Responsible for the continuous monitoring of all services that the client is linked.
  • Responsible for updating the EMR.
  • The case manager shall provide transportation for clients as needed along with assisting them in referrals to transportation agencies.
  • Provide quality, sufficient, and appropriate services and for maintaining high professional standards.
  • This is a community-based position, so work will be performed outside of an office setting on a regular and consistent basis up to 100%
Requirements:

Qualifications for the position are:

  • Bachelor’s degree in behavioral health or a related field
  • 1 year experience in the Behavioral Health field
  • 1 year Case Management experience
  • Must have a valid driver's license with 2 points or less.

Schedule:

  • Monday - Friday
  • 7:00 a.m. - 3:00 p.m.

Salary:

  • From $21/hr. commensurate with experience
