Job description

The Community Health Worker (CHW) provides a link between patients at the Agency's contracted hospital systems and available community resources with the goal of reducing negative health outcomes. The CHW is expected to conduct outreach contacts, in-home assessments, data entry, data tracking based on programmatic reporting requirements and other duties as assigned. The CHW will communicate and collaborate with team members, various AgeWays departments, Vendor Agencies, community resources and Hospital system staff.

Duties Include:

  • Contacts participants and/or family, to complete outreach, schedule/reschedule assessments. Enter member information and related activities into internal/Hospital EMR, maintain participant files according to the established procedures. Contact patients, vendors, community resources to initiate services.
  • Demonstrates critical thinking, problem solving and conflict management skills when receive incoming calls regarding service issues, grievances, or complaints from participants, Hospital systems, and other stakeholders.
  • Monitor Outlook and internal/Hospital EMR for updates. Respond to all documentation notifications from vendors, internal team members, and other stakeholders. Process information/requests per program quality standards.
  • Maintain data tracking spreadsheets to monitor service quality. Prepare data reports for various touch base meetings.
  • Participate in staff development activities, meetings, and adhere to agency, department, and program standards. Assist with training new staff.
  • Other duties as assigned

QUALIFICATIONS:

  • High School/GED, Health & Human Services or related field. Associates Degree preferred
  • 1-2 years preferred experience, in office administration,health/human services or related field
  • Excellent interpersonal, written and verbal communication, and organizational skills
  • Effective time management skills, including the ability to meet deadlines
  • Strong analytical, critical thinking, operations quality management, process improvement, problem-solving, and excellent customer serviceskills. Proven ability to foster and maintain open, collaborative, and constructive relationships with co-workers andleadership to achieve departmental and operational results
  • Detail oriented; high level of accuracy; extensive analytical skills; and ability to maintain a high level of productivity
  • Experience working with seniors and adults with disabilities (AWD) is preferred
  • Intermediate Microsoft Office skills
  • Ability to learn and effectively utilize EMR and various databases
  • Community Health Worker Certificate preferred
