Job description

Salary: Salary range of $20.44 - $31.34 p/hour + Benefits

JOB STATUS: Full-time (40 hours), 12 months per year

LOCATION:Marshall, MN


Connecting Communities, Empowering Lives! Passionate about making a difference? Join us
as a Community Connector and play a vital role in engaging, educating, and assisting individualsin accessing essential programs and serviceswithin UCAP and the broader community!

WHAT YOU'LL DO:

Engage & Educate: Conduct outreach,presentations, and events to promoteUCAP programs.
Facilitate Access: Guide individuals throughscreening and enrollment for services.
Champion Health Equity: Strengthenpartnerships with healthcare and publicservices to support RIM communities.
Develop Partnerships: Collaborate with BIPOC organizations for culturally responsiveoutreach.
Support Participants: Help individualsnavigate services, understand rights, andcreate action plans.
Promote Collaboration: Build a strong referralnetwork with community groups, schools, and
businesses.


SKILLS TO WOW US:

Strong verbal and written skills to effectivelyengage diverse communities.
Ability to establish trust, build relationships, andwork collaboratively.
Understanding of diverse backgrounds with acommitment to equity and inclusion.
Ability to manage multiple tasks, documentactivities, and meet deadlines.
Creative and adaptive approach to connectingindividuals with resources.

EDUCATION AND EXPERIENCE:
Minimum: Health Services Worker CertificationOR a total of five (5) combined years of
education + relevant experience
Preferred: Bachelors degree in Human Services(or related field) with two (2) years of human
services experience
