Job Summary:

We are seeking a talented and experienced Registered Nurse to join our assessment team as a Minimum Data Set (MDS) Coordinator. As a key member of our care team, you will be responsible for conducting admission/skilled assessments to develop individualized care plans, ensuring our residents receive the highest level of personalized attention and care.

Responsibilities:

• Conduct thorough assessments of residents, including MDS assessments, to identify care needs and develop individualized care plans
• Collaborate with interdisciplinary team members to ensure effective care delivery and coordination
• Monitor and evaluate resident outcomes, adjusting care plans as necessary to ensure optimal health and well-being
• Maintain accurate and timely records of resident assessments, care plans, and outcomes
• Participate in quality improvement initiatives to enhance the overall quality of care and services
• Will be part of the on-call rotation with weekends as needed

Requirements:

• Current Registered Nurse (RN) licensure in the State of Missouri, or current multistate license
