South Haven Health & Rehabilitation is seeking a Registered Nurse (RN) to serve as our Resident Assessment / MDS Coordinator (Resident Assessment Coordinator). This position offers a Monday-Friday day shift schedule with a primary focus on resident assessments, care planning, and coordination of clinical documentation.

Position Summary
The Resident Assessment / MDS Coordinator leads the interdisciplinary assessment and care planning process for residents. Responsibilities

Qualifications
  • Current Registered Nurse (RN) license in good standing
