Position Overview

The Community Health Network is seeking a dedicated Full-Time Licensed Clinical Social Worker (LCSW) to join our team, working remotely from the Naperville, Illinois area. Key Responsibilities
  • Conduct thorough assessments of clients' social, emotional, and psychological needs, employing standardized tools to facilitate your evaluations.
  • Provide individual and group counseling sessions, utilizing evidence-based therapeutic approaches tailored to the unique needs of clients—this may include techniques such as cognitive-behavioral therapy (CBT) or mindfulness-based stress reduction.
  • Create and implement personalized service plans with specific, measurable objectives to address clients' needs, ensuring alignment with their goals for therapy.
  • Actively advocate for clients' rights and access to essential resources, effectively navigating complex social service systems on their behalf.
  • Engage in comprehensive program development, evaluation, and grant reporting, collaborating with team members to assess the impact of interventions and suggest improvements.
  • Participate in interdisciplinary team meetings and community outreach initiatives, sharing insights and innovations to enhance service delivery.
  • Document client interactions meticulously in our electronic health record system, adhering to confidentiality and regulatory standards.
Patient Population / Client Focus

As an LCSW at Community Health Network, you will serve a diverse client base, including individuals and families facing socioeconomic challenges, mental health disorders, trauma recovery, and marginalized communities. Your role will involve addressing immediate needs while also working to empower clients with coping strategies, resources, and a sense of agency in their lives.

Qualifications
  • A Master’s degree in Social Work from an accredited institution is required.
  • An active state license as a Licensed Clinical Social Worker (LCSW) is mandatory for this position.
  • Demonstrated understanding of social justice principles, trauma-informed practices, and advocacy is essential to fulfill your duties effectively.
  • Strong communication skills—both verbal and written—are necessary to foster relationships with clients and community partners.
  • High levels of empathy and organizational abilities are critical to manage multiple cases effectively.
Preferred Experience

Compensation and Benefits
