If you're a home health RN who wants steady, predictable hours without giving up meaningful, hands-on patient care, this PRN Case Manager role in Medford is worth a look. You'll work Monday through Friday, 8:00 a.m. to 5:00 p.m., covering Medford and the surrounding area, with the flexibility that comes with a PRN schedule.

What Your Week Looks Like

Your days center on your patients and their families. Expect to deliver direct skilled care, provide hands-on education and emotional support to patients and caregivers, and coordinate visit scheduling and documentation to keep everything compliant. On-call rotation, admissions, and urgent response may also come up as part of the role.

What We're Looking For
  • Graduate of an approved nursing program, currently licensed as an RN in the state of practice
  • At least one year of RN experience
  • Current CPR certification
  • Valid driver's license with insured, reliable transportation for travel between locations
