Job description

Summary: Golden Care of Northeast PA, Inc. is seeking dedicated Home Health Aides (HHA) or Certified Nursing Assistants (CNA) to join our compassionate team. Under the general supervision of clinical management, the HHA or CNA will provide general aide care to assigned patients, ensuring high-quality care and compliance with all relevant standards and regulations. This role may include on-call duties for after-hours work.

General Home Health Aide Role

  • Provide generalized aide care tasks to assigned patients.
  • Report patients' physical and emotional status, competency, home environment, safety factors, and available family support to team members.
  • Offer information on patient rights, reimbursement procedures, and patient/family rights.
  • Observe patients for changes in condition and communicate/coordinate care with attending physician and clinical management.
  • Assist patients with activities of daily living and support their efforts toward self-sufficiency and independence.
  • Maintain accurate patient clinical records and reports.
  • Participate in on-call rotation as necessary.
  • Perform other related duties as assigned by management.

Qualifications:

  • Active HHA certification in good standing in Pennsylvania. At least one year of patient care experience preferred, but not required.
  • Currently licensed as a Certified Nursing Assistant in the state.

Skills and Abilities:

  • General care skills, observation skills, and communication skills.
  • Knowledge of home health care and government regulations.
  • Good physical and mental health.
  • Caring attitude, empathy, tact, and patience.
  • Ability to solve practical problems and interpret various instructions.

Physical Demands:

  • Requires moderate to heavy physical exertion, including driving to patient homes, assisting in transfer activities (wheelchair, bed, tub, toilet), and supporting individuals in ambulation therapy.
  • Regularly required to use hands, talk, and hear; frequently required to stand, walk, reach, stoop, kneel, crouch, or crawl.
  • Occasionally required to sit and lift/move over 100 pounds.
  • Vision requirements include close vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
