Job description

Clinical Social Worker in the Collaborative Care program for the Child/Adolescent Department is responsible to provide brief interventions using evidenced based techniques such as Behavioral Activation, Problem Solving treatment, Play Therapy or other appropriate treatments in primary care settings. Work collaboratively with PCP and psychiatric consultant. Supports psychotropic medication management prescribed by PCP's focusing on treatment adherence, side effects and other complications and effectiveness of treatment. Participates in regularly scheduled caseload consultations with the Psychiatric Consultant and communicates resulting treatment recommendations to the patient's PCP, focusing on patient's new treatment or who are not improving as expected.
Responsible for documentation in the EMR and track patient's follow-up/clinical outcome in CMTS registry. Facilitate referrals for clinical indicated services outside of primary care clinic. Facilitate patient engagement and follow up care. Complete relapse prevention plan with patients who are in remission.

Education: Master's Degree in Social Work, Mental Health Services.
License/Certification/Registration(s) Required: Licensed Clinical Social Worker, Licensed Master Social Worker, Licensed Mental Health Counselor, or Certified Counselor or Licensed Marriage/Family Therapist or Licensed Psychologist, Licensed NP.
Experience/Skills Requirement: 3+ years relevant experience working with child/adolescents' patients from underserved populations who have co-occurring behavioral health and physical problems.
Bilingual (Spanish/English) required.
Computer literate with data entry capabilities. Knowledge of healthcare and chronic diseases. Knowledge of Social Service programs and service plans.
