About the Role

We’re looking for a dedicated Behavioral Health Care Manager-Collaborative Care to join our team and help us continue delivering the level of care our patients and families deserve. In this role, you will help to identify and manage patients’ mental and behavioral health concerns in conjunction with the patients’ primary care provider and psychiatric consultant. The behavioral health care manager provides various evidence-based treatment modalities, including solution-focused brief therapy and motivational interviewing, as well as supportive counseling and psychoeducation. The goal of behavioral health treatment within the primary care sphere is for patients to feel encouraged to implement and maintain their goals related to their mental and physical health and well-being.

What You’ll Do

  • Works collaboratively with primary care provides to identify and treat mental and behavioral health concerns, such as depression and anxiety, as well as assist in developing behavioral health change plans to promote medical compliance.
  • Provides psychoeducation and supportive counseling through evidence-based treatment modalities, such as behavioral activation and problem-solving treatment.
  • Screen and assess patient for common mental health conditions and facilitate patient engagement and follow-up care.
  • Support psychiatric medication management, in conjunction with psychiatric consultants and as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment. Provide or facilitate referrals to additional care options, when clinically appropriate.
  • Participate in regularly scheduled case consultation with the psychiatric consultant and act as a liaison between the primary care provider and psychiatry. Consultation should focus on patients new to treatment as well as those whose symptoms are not improving despite behavioral/psychiatric intervention.
  • Maintain consistent, clinically accurate progress notes related to patient progress and treatment recommendations in hospital EHR as well as state approved registry.
  • Develop and complete relapse prevention self-management plans when clinically appropriate.

What You Bring

  • Graduate degree from an accredited master’s program in a mental health field.
  • NYS license to practice as a mental health professional (LCSW, LMHC, or RN-BSN with psychiatric specialization). Conditional NYS license to practice (LMSW, MHC-LP) may also be considered.
  • Documented training in brief psychosocial interventions, such as problem-solving treatment.
  • Experience in case management, counseling, or mental health services required.
  • Knowledge of/experience with the Collaborative Care model preferred.
  • Proficiency in brief, evidence-based psychosocial interventions, such as problem-solving treatment.
  • Excellent collaboration skills, working effectively with interdisciplinary care teams.
  • Experience in case management, counseling, and/or mental health services, ideally with knowledge of the Collaborative Care Model.
  • Strong time management skills.
  • Excellent written and verbal communication skills.
  • General computer knowledge with the ability to manage clinical documentation responsibilities within an electronic medical record.
  • Flexibility and adaptability to both in-person and remote work settings.
