Primary Care Physician โ€“ Pod Leader Position: Primary Care Physician โ€“ Pod Leader Industry: Medical Center (Inpatient) Job Type: Full-Time Experience: 2โ€“5 years Location San Jose/ North Hollywood, CAEducation: Doctorate DegreeSpecialties : Internal Medicine/Family Medicine/Emergency Medicine/Geriatrics Position Overview We are seeking experienced Primary Care Physicians to provide comprehensive, community-based care to vulnerable senior populations. This role works within an interdisciplinary team structure and allows physicians to develop meaningful, long-term relationships with a smaller patient population. The ideal candidate will have experience in primary care, preferably with geriatric patients, and be comfortable collaborating with and leading members of an interdisciplinary care team.

Key Responsibilities
  • Conduct comprehensive in-person patient assessments.
  • Lead and collaborate with the interdisciplinary team (IDT).
  • Develop individualized care plans for patients.
  • Coordinate acute, short-term, and long-term care placement and discharge.
  • Provide comprehensive primary care without hospital rounds.
  • See an average of approximately 8 patients per day.
  • Participate in a rotating on-call schedule by phone.
  • ScheduleMondayโ€“Friday8:00 AMโ€“5:00 PM
  • Weekends generally off
  • Rotating phone-based on-call coverage a few nights or weekends per month
Requirements
  • Successful completion of an ACGME-accredited residency.
  • Master's or Doctorate degree.
  • Active, unrestricted physician license in the applicable state.
  • State CDS certification.
  • Active DEA registration.
  • Board certification preferred.
  • Eligible to enroll and provide services to Medicare and Mediโ€‘Cal participants.
  • CPR and First Aid certification required within 180 days of hire; certification upon hire preferred.
Ideal Candidate
  • 2โ€“5 years of primary care experience.
  • Strong background in geriatric care preferred.
  • Experience working within an interdisciplinary team (IDT).
  • Experience leading or supervising Nurse Practitioners or Physician Assistants is preferred.
