Description:
Plans and administers medically prescribed occupational therapy treatment programs for patients.
Restores function, relieves pain, and prevents disability following disease, injury, or loss of body part.
Candidate will work three days in one location and two days in another location.
Locations are 17 miles apart.
Dress code is business casual or solid scrubs.
Requested time off during the assignment must be noted and verified at the time of submission.
No more than five days of requested time off will be accepted; fewer days are better.
Traveler must have a clean driving record and their own insurable vehicle.
Traveler's permanent address must be included in their profile.
All scheduling requests must be made at the time of submission.
Requests after the offer may lead to a rescinded offer.
Profile requirements include work history and a skills checklist for occupational therapy current to one year.
A supervisory signed reference from within the past two years of employment is required.
