Description

For over 125 years, Calvary Hospital and Calvary @Home located in the Bronx, New York has been dedicated to the palliative care of advanced cancer and other terminally ill patients and their families. Through a caring, compassionate, philosophy of non-abandonment, the hospice provides a full spectrum of palliative services, including emotional, spiritual, and physical care. The hospice receives patients from referring institutions throughout New York City and the entire tristate area. When you come to Calvary Hospital, you're not just getting a job, you're joining a family.

Requirements: Certifies eligible patients for hospice services, orders medications, equipment, and other services as needed. Leads the interdisciplinary team in the ongoing care of patients.  Visits patients as needed.

ESSENTIAL JOB FUNCTIONS:

  • Completes certification/recertification of terminal illness as needed for eligible patients.
  • Orders medications, equipment, and other services as needed for the care of admitted patients.
  • Documents in the patient's clinical record as needed visits (face-to-face, etc.) or other communications.
  • Coordinates care as needed with patient's Attending Physician.
  • Serves as the patient's Attending Physician as needed.
  • Participates in the interdisciplinary group (IDG) (meetings, etc.) to monitor the patient's status and modification of the plan of care.
  • Takes calls from field staff as needed to support the care of the patient.
  • Liaises as needed with staff at hospitals for admitted patients while on hospice.
  • Certifies death certificates as needed.
  • Participates in the on-call physician rotation for admission of patients.
  • Provides education to IDG team and hospice staff as needed.
  • Participates in chart audits as needed.
  • Maintains credentials to remain on Calvary Hospital Medical Staff.
  • Be able to perform physical activity that involves no more than 20 lbs.
  • Works within legal and ethical practice standards to his/her position as well as Calvary's  Code of Conduct.  Complies with appropriate policies and procedures and brings potential compliance matters to supervisor or compliance officer.
  • Exhibits an ability and attitude relative to the spirit of caring and tradition at
  • Calvary Hospital.

Status: Full-Time

Salary: $250K plus bonus opportunity

Qualifications: 

  • Current New York State license.
  •  DEA registration
  •  Possess a valid driver's license.
  •  Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions.
