Job Description
Job Title: Registered Nurse (RN)
Profession: Nursing

Experience: A minimum of one year experience as an RN, either in acute care hospital, oncology, long-term care, hospice, or home health.
License: Current license to practice nursing in the state of employment.
Certifications: BLS (American Heart Association).
Must-Have:
- Knowledge of terminally ill patients and their families.
- Understanding of hospice philosophy.
- Ability to apply knowledge of the special needs of hospice patients and families.
- Excellent patient assessment skills.
- Knowledge of roles of all disciplines providing hospice services.
Description:
The Registered Nurse (RN) will provide direct patient care services to hospice patients.
Supportive care will be provided to the patient's family.
The RN will manage an interdisciplinary group consisting of support staff to enhance the quality of hospice care.
Case management of all assigned patient clinical records will be performed.
Quality care will be assured and documented in accordance with the Plan of Care.
The RN will perform initial and ongoing assessments of patients.
Assistance in developing and implementing the Hospice Plan of Care prescribed by the physician will be provided.
Collaboration with other interdisciplinary team members will focus on providing all-inclusive services including psychosocial, emotional, and spiritual support.
Any change in the patient's condition will be reported to the attending physician.
The RN will maintain accurate and updated medical records, including all interdisciplinary notes and assessments.
Infection control measures and standard precautions will be practiced.
The RN may monitor scheduling of staff.
Participation in the Quality Assessment and Performance Improvement Program is required.
There may be a request to participate in the agency on-call schedule.
The RN will promote core values consistently.
Completion of required compliance training is necessary.
Other activities as assigned should be expected.
Staff meetings will be attended regularly.
A high level of knowledge regarding the management of pain and symptoms in the dying process must be maintained.
Routine supervisory visits with all team members will be performed monthly.
Ongoing review and follow-up on assigned patient clinical records will ensure quality services are provided.
Close contact will be maintained with the patient's family to provide information and support.
The RN will have primary control and professional management of each patient.
Acting as a liaison between the physician, patient/family, and hospice team is essential.
Education of the patient, family, or caregivers regarding the needs identified in the Plan of Care is a critical responsibility.
This includes instruction on medication administration, dressing changes, and care of the terminally ill patient.
