Job Description
Job Title: Physical Therapist - Home Health
Profession: Therapy/Rehabilitation

Experience: Minimum 1 full year of Adult Home Health experience
License: Must have active license in hand
Certifications: CPR, BLS
Must-Have:
Must have reliable transportation, a current driver's license, and agency-required liability insurance.
Must be willing to travel up to 75 miles from home clinic.
Must have experience with HomeCare HomeBase EMR.
Must be comfortable accepting all patients on the schedule regardless of distance between appointments.
Must understand the expectation of completing all documentation within 24 hours of patient visits.
Must be open to on-call requirements as they vary by job and location.
Must understand there are no guaranteed hours with this contract.
Description:
This position involves providing home visits to patients.
The average number of patients is 5-10 per day.
Daily meetings will take place periodically in the office.
Each type of visit and activity holds a different point value, with productivity expectations set at a minimum of 30 points per week.
Contractors will be assigned 5-10 patients per day that equate to roughly 6-8 points.
There may be variability in weekend requirements depending on the location.
Experience in home health is required; first-time home health contractors will not be accepted.
Mileage will be reimbursed at the IRS Standard rate, with compensation provided from patient home to patient home.
Travelers must be proficient in OASIS and demonstrate recent use.
This role requires travel to and from patient homes daily.
Contractual obligations include accepting patient assignments that may result in longer driving distances.
It is essential to maintain complete and timely documentation, as delays can impact patient care and billing.
