Job description

POSITIONย OVERVIEWย 

The Diversion Hub is a comprehensive, one-stop network dedicated toย assistingย justice-involved individuals in Oklahoma City by harnessing the power of multiple support agencies through combined and synchronized services. Our goal is to provide life-stabilizing resources while empowering individuals to reduce their encounters with the criminalย legalย system through enhanced support services, including case management, dataย integrationย and coordinated communication under one roof.ย The Diversion Hub CO-OP Recovery Navigator, or Court Ordered Outpatientย Programย Recovery Navigator,ย will provide direct program navigation to clientsย participatingย inย Oklahoma County CO-OP.ย ย 

RESPONSIBILITIESย 

Theย CO-OP Recovery Navigatorย will focus on supporting the execution of the Diversion Hub's programs and mission. The responsibilities ofย this roleย include:ย 

  • Work closely withย Publicย Defenderโ€™s Office on early identification and referral to CO-OP.ย 

  • Meet CO-OP participants in custody with Assistant Public Defender for Needs Assessment.ย 

  • Coordinate with the jail for all CO-OP participants to be released with prescriptions for mental health medications and ensure they are filled the same day.ย 

  • Work withย RideCareย for transport to treatmentย appointment.ย 

  • Attendย initialย appointmentย withย clientย to ensureย appropriate coordination/scheduling.ย 

  • Transport client back to Diversion Hub for emergency needs (food stamps, ID,ย SoonerCare, housing, hygiene,ย etc).ย 

  • Coordinate for transportation to follow up appointment(s) with treatment provider.ย 

  • Provide outreach in the community for continued engagement (shelters, Day Center,ย etc).ย 

  • Ongoing liaison and advocate with treatment provider to ensure all needs are met.ย 

  • Participate fully with CO-OP Treatment Teamย staffings, act as part ofย CO-OP team.ย 

  • Work with CO-OPย programย to increase engagement and successful participation.ย 

  • Adhere to agency code of ethics and professional standards includingย maintainingย strict client confidentiality.ย 

  • Attendย all staff development,ย trainingsย and meetings asย required.ย 

  • Any other duties within the scope,ย spiritย and purpose of the job.ย 

EDUCATION AND EXPERIENCEย 

  • 1 toย 3+ years of experience in social work, communication, criminal justice, politicalย scienceย or similar human service area.ย 

  • PRSS Certificationย preferredย 

ADDITIONAL JOB REQUIREMENTSย 

  • Clearance of backgroundย 

  • Must have reliable transportation, a valid driver's license, and current automobile insurance.

PHYSICAL DEMANDSย 

While performing the duties of this job, the employee must be able to sit over long periods of time and be able to talk, hear, sit, stand, walk; use hands to finger, handle or feel objects, tools, or controls; reach with hands and arms; and stoop, kneel, crouch, or crawl climb stairs, lift and or move items of up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. Must have excellent command of English language and grammar, both verbal and written. Must be able to manuallyย operateย and use a computer. Must be able to clearly hear and understand telephone conversations.ย
