Job description

JOB DEFINITION:
Work with the CN Guidance FACT team to deliver services to those individuals who are
diagnosed with severe mental illness and whose needs have not been met by more
traditional service delivery approaches.
EDUCATIONAL REQUIREMENTS:
A Bachelor's degree or higher in social work, psychology, rehabilitation counseling, or related field. Licensure with the NYS Education Department in a specific field is preferred.
ADDITIONAL REQUIREMENTS:
  • Current New York State Driver's License
  • Clean DMV Record
  • Proof of Motor Vehicle Insurance

EXPERIENCE REQUIRED:
Two years' experience working with the chronically mentally ill (Masters level) recommended. Three years' experience working with the chronically mentally ill (Bachelor's level) recommended. Competency in vocational rehabilitation must be demonstrated by at least one year of training and/or experience in integrated mental health and substance assessment and treatment or vocational assessment.
DUTIES AND RESPONSIBILITIES:
  1. Responsible for providing Assessment and Service Planning, Quality Assurance and Improvement Processes, Utilization Review, incident reporting and coordination between ACT team(s) and other systems
  2. Engage in performing assessment, case management, handling crisis intervention and discharge planning
  3. Maintains case records and completing documentation on time and in a matter in compliance with federal, state and agency regulations
  4. Provides direct, telehealth, face-to-face and/or collateral services to members either in the FACT office or in the community in places and situations where problems arise
  5. Must be available (along with other members of the FACT team) on an as needed basis 24 hours a day, 7 days a week, including responsibility for on-call coverage
  6. Assists members in developing appropriate expectations from treatment
  7. Strengthens the ability of the member to understand and utilize therapy and find resources after treatment has ended through proper transition and discharge planning
  8. Exchanges information with other agencies that benefits the member or ensures compliance with regulations and coordination of services
  9. Utilizes and understands the value of supervision, which increases practice skills, promotes better services for the member and preserves accountability to the Agency
  1. Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress
  2. Complete QI Training during onboarding and participate in QI refresher training as needed.
  3. Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences. A Culture that makes better decisions, drives innovation, and delivers better business results.
  4. Adherence to all safety protocols and procedures to ensure a safe working environment for all employees. Demonstrate a commitment to maintaining a safe work environment by following established safety guidelines.
