Job Description
We are seeking a dedicated Nurse Practitioner (NP) to provide high-quality care in post-acute settings. In this role, you will support patients during their recovery following hospital discharge, helping to reduce readmissions and improve overall outcomes. This position offers a structured weekday schedule, autonomy in practice, and a collaborative team environment.
Key Responsibilities:
  • Conduct rounds in assigned facilities on a consistent weekday daytime schedule.
  • Perform focused patient assessments to monitor recovery, manage pain, wounds, and functional decline.
  • Collaborate with physicians, therapists, and facility staff to develop individualized care plans.
  • Complete documentation using an electronic health record (EHR) system (with remote flexibility).
  • Identify and address readmission risks and gaps in post-acute care.
  • Ensure compliance with applicable clinical and regulatory standards.
