About the Role

The Supports Coordinator Assistant - Adults helps individuals access services, resources, and supports that promote independence and community inclusion. This role works under the supervision of a Qualified Supports Coordinator or Manager and adheres to Person-Centered Planning principles and regulatory requirements.


Key Responsibilities

  • Assist with implementing Individual Plans of Service (IPOS)
  • Link, coordinate, and monitor services and supports
  • Help individuals maintain entitlements (Medicaid, SSI, etc.)
  • Support crisis planning and resolution
  • Coordinate health and safety strategies, including discharge planning
  • Promote community inclusion, self-advocacy, and relationship building
  • Assist with housing, transportation, and employment resources
  • Complete timely documentation and maintain compliance with agency and funder requirements

Qualifications

  • Education:
    • Bachelorโ€™s degree in Human Services (eligible for advancement to Supports Coordinator after one year)
    • OR Bachelorโ€™s degree in non-human services field
    • OR Associate degree or 60 credit hours plus 2 years of experience with individuals with I/DD
  • Strong communication, organizational, and computer skills
  • Valid Michigan driverโ€™s license, insurance, and acceptable driving record

Additional Requirements

  • Extensive travel within Wayne County; reliable transportation required
  • Ability to work flexible hours and maintain documentation standards
  • Completion of job-specific training provided by CLS
