The Lemire Intermediate Resource (IR) is a warm, family-friendly environment dedicated to supporting nine adults living with intellectual disabilities (ID), autism spectrum disorder (ASD), and severe behavioral disorders (SBD). Our mission is to foster their growth and independence in a safe and caring environment. Job Description: Reporting directly to the Executive Director, you will be responsible for the clinical coordination of services provided to our residents. Your role will be essential in ensuring the quality of interventions and the well-being of each individual. Responsibilities: Development, implementation and monitoring of individualized intervention plans (PII) as well as the service offer for each user according to the Instrument for Determining and Classifying Support or Assistance Services (IDC). Supervision and support of the intervention team. Coordination of health services and activities. Managing crisis situations and incidents calmly and efficiently. Regular communication with families and partners. Participation in the continuous improvement of practices and procedures. Ensure compliance with current standards and regulations. Requirements: University training in social work, psychoeducation, psychology or related field. Significant experience in clinical coordination, ideally with DI-ASD-TGC clients. In-depth knowledge of the issues and interventions related to this clientele. Leadership, communication and problem-solving skills. Ability to work in a team and independently.
