Home Dialysis Nurse (PRN / Per Patient Model)

Premier Dialysis Remote with Travel to Patient Homes (Approx. 75%)

Position Overview

Premier Dialysis is seeking a highly motivated and independent nurse to support the onboarding and ongoing management of home dialysis patients. This role is ideal for nurses looking to supplement their income while maintaining flexibility and autonomy.

This is a performance-based, per-patient role โ€”there are no set hourly requirements. Your focus is simple: deliver high-quality care on your schedule and the patient's schedule.

Key Responsibilities
  • Onboard new home dialysis patients in their residences
  • Perform routine patient assessments and clinical evaluations
  • Coordinate and review lab work
  • Manage medication requisitions and administration as appropriate
  • Ensure accurate and timely documentation
  • Communicate regularly with Lead RN regarding patient status and needs
  • Support patient education and retraining as needed
  • Maintain compliance with all clinical and regulatory standards
Work Structure
  • Flexible Schedule โ€“ No hourly requirements
