Position Overview:
We are seeking a dedicated and compassionate Bachelor of Social Work (BSW)/Master of Social Work (MSW) or Licensed Master Social Worker (LMSW) to serve in a multifaceted role combining community outreach, case management, and clinical therapeutic services. This position is ideal for a professional who is passionate about holistic care, advocacy, and empowering individuals and families within the community.

Key Responsibilities:

  • Serve as a Community Liaison, building and maintaining strong relationships with community partners, agencies, and stakeholders
  • Conduct outreach to connect individuals and families with appropriate resources and services
  • Provide comprehensive case management, including assessments, care planning, referrals, and ongoing support
  • Deliver clinical therapeutic services (individual, family, and/or group counseling) in accordance with scope of practice and licensure
