Position Summary

The Licensed Clinical Social Worker (LCSW) provides high-quality, patient-centered behavioral health services to individuals and families at Crossroads Family Care. This role delivers assessment, diagnosis, and therapeutic interventions while addressing social determinants of health and supporting integrated care. The LCSW promotes emotional wellness, crisis stabilization, and continuity of care in alignment with organizational standards and best practices.



Essential Duties & Responsibilities Clinical Care & Counseling Services
  • Conduct behavioral health assessments, psychosocial evaluations, and diagnostic screenings.
  • Provide individual, family, and/or group therapy using evidence-based practices.
  • Develop and implement individualized treatment plans.
  • Provide crisis intervention, safety planning, and risk assessments.
  • Address social determinants of health impacting patient outcomes.
Care Coordination & Patient Support
  • Coordinate care with medical, dental, and behavioral health teams.
  • Connect patients to community resources and support services.
  • Assist with referrals for housing, financial assistance, transportation, and other needs.
  • Promote continuity of care and patient engagement.
Documentation & Compliance
  • Maintain accurate, timely, and complete documentation in the EHR.
  • Ensure compliance with HIPAA, HRSA, and licensing requirements.
  • Participate in audits, chart reviews, and quality assurance activities.
  • Follow all policies related to confidentiality, safety, and mandatory reporting.

Qualifications

Required

  • Experience in an FQHC or integrated care setting.
  • Experience with EHR systems.
