• Conduct thorough clinical assessments to evaluate patient needs and create effective treatment plans tailored to individuals' circumstances.

• Maintain accurate and timely clinical documentation in electronic health record systems to ensure seamless communication within a multidisciplinary team.

In this role, you will cater to a diverse patient demographic, ranging from individuals coping with chronic illness to families looking for guidance and support during challenging times. • Master's degree in Social Work from an accredited institution.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of New York.
• Knowledgeable in evidence-based therapy modalities, particularly those focusing on solution-focused and cognitive-behavioral interventions.
• Exceptional communication skills and the ability to build rapport with patients and their families.

• Experience working in a hospice or palliative care setting is highly desirable.
• Previous involvement in a multidisciplinary healthcare team.
• Familiar with electronic health record (EHR) systems and telehealth platforms.

• An attractive salary range of USD 80,000 – 100,000 per year, commensurate with experience and qualifications.
