Job description

Job Title

Answer and handle all client intake calls, complete intake assessments, determine baseline of client eligibility, direct client inquiries, assist in coordinating assessors and provide referrals.

Main Responsibilities

Complete intake assessment on all prospective client calls for Meals on Wheels (MOW)

Secure information from callers such as medical, psychological, and social factors contributing to client's situation and direct inquiries to client services staff.

Provide referral information to callers needing assistance from other organizations (including United Way). Refer eligible clients to other IM programs and services (such as Animeals).

Maintain contact with assessors. Assist in relaying client information and needs to assessors when needed.

Maintain client services data (number of re-/assessments completed, attrition, waiting list, etc.)

Fill out Change of Status Forms when needed.

Enter inquiries into and maintain MOW waiting list in client management system, Caseworthy ServTracker.

Determine client's initial eligibility for IM services using discretion and independent judgment with respect to the MOW qualifications and explained client situation.

Code intakes to the appropriate route.

Assist in developing flyers for MOW clients.

Compile records and prepare reports as needed from the SERVtracker system.

Act as a backup to the Client Services Coordinator.

Document and address client concerns.

Other duties as requested by supervisor.

Qualifications, Skills & Experience

High school diploma or equivalent is required. Associate degree or higher is preferred.

Minimum two years of customer service and clerical experience.

Bilingual (English/Spanish).

Computer proficiency required. Experience using Microsoft Office (esp. Excel) and databases.

Must be dependable and have good organizational and interpersonal skills.

Strong and proven attention to detail, interpersonal and computational skills.

Detail oriented with good organizational skills and effective verbal and written communication skills.

Must have reliable transportation, valid driver's license, and automobile liability insurance.
