GhostRx Inc. is seeking full-time Community Living Center (CLC) Registered Nurses to provide professional medical services and quality care to our Veteran population at the Louis Stokes Cleveland VA Medical Center (LSCVAMC) in Cleveland, Ohio. CLC Registered Nurses are required to deliver high-quality, compassionate, and consistent patient care within a 24/7 long-term care, skilled nursing, and rehabilitation environment, supporting Veterans who require post-acute recovery, short-term rehab, respite, placement, or medical care that cannot be safely delivered at home.

Qualification and Requirements

minimum of 2 years experience, BLS

Responsibilities

  • Provide short-term, post-acute skilled nursing and rehabilitation services to patients who require intensive clinical care and therapy following hospitalization, surgery, or acute illness.

  • Perform comprehensive patient assessments, medication administration, IV therapy, and lab draws.

  • Manage central and peripheral line IV medications, tube feedings, foley catheters, wound care, and trach care.

  • Assist Veterans with activities of daily living and coordinate care through the interdisciplinary team.

  • Manage admissions, discharges, and internal and external patient transfers.

  • Delegate tasks appropriately to assistive personnel and document all care in the electronic medical record.

  • Coordinate patient transport and execute emergency response procedures.

  • Float between CLC inpatient units based on staffing needs.

  • Collaborate with an interdisciplinary team to provide comprehensive care to Veterans completing rehab, with the goal of returning to their prior functional status after an acute medical stay.

  • Provide comprehensive care to Veterans requiring more respiratory-focused care than can be supported in the home environment.

  • Utilize the nursing process to provide comprehensive care to Veterans admitted for rehab, IV antibiotic management, and simple to complex wound care.

  • Provide comprehensive holistic care to Veterans admitted for respite, placement needs, short-term rehab, or medical care that cannot be safely given at home.
