Our Purpose
Inspired Home Health is dedicated to providing compassionate and personalized in-home health care, ensuring the health, comfort, and independence of our clients in the comfort of their own homes.
Role Summary
As a PRN Social Worker supporting home health, you will help clients and families navigate psychosocial needs and community resources to promote safe, appropriate care planning-while working fully remote. Key responsibilities include completing psychosocial assessments, identifying barriers to care, coordinating referrals to community services, supporting advance care planning discussions as appropriate, and documenting thoroughly within required timeframes.
Required qualifications/skills:
- BSW or MSW from an accredited program
- Current Arizona social work licensure (as applicable)
- Home health/hospice or medical social work experience preferred
- Strong clinical judgment, communication, and documentation skills
- Ability to manage PRN caseload needs with reliability in a remote setting
