Department/Location: White River Pediatrics
Reports To: Practice Manager
Classification: Non-Exempt | Full Time
Requires: High School diploma or GED equivalency is preferred.
Open Door Health Services is seeking a full-time Community Health Worker (CHW) at 2600 W. White River Blvd. in Muncie, Indiana.
Overview
This position will lead and coordinate daily operations in a variety of patient care activities. The individual will assist in the delivery of health care and patient care management in accordance with scope and licensure and will take an active lead in facilitating identified performance improvement activities within the department. Will uphold ODHS Mission, Vision, and Values.
As a Federally Qualified Health Center, Open Door Health Services provides comprehensive primary, urgent, preventive, and select specialty services and social support programs for individuals and families, regardless of their ability to pay. Our patients represent diverse backgrounds, life experiences, capacities, and orientations.
Location
This position is located at 2600 W. White River Blvd, Muncie, IN.
Roles amp; Responsibilities
In this role, you will:
  • Connect patients to internal and community resources, improving access to care and social services.
  • Act as a liaison between patients, healthcare providers, and external agencies to coordinate support.
  • Maintain knowledge of social service programs and ensure compliance with policies, laws, and ethical standards.
  • Support patient independence by teaching skills, providing guidance, and following up on progress.
  • Document case activity, collaborates with teams, and address patient needs across financial, social, emotional, and health-related areas.

Qualifications
  • High School diploma or GED equivalency preferred.
  • Experience in a health care setting and/or in an office preferred.
  • Previous experience as a Community Health Worker, or similarly related role, preferred.
