Job description

Schedule: Full-time and part-time opportunities available Monday-Friday  schedules (no weekend, holiday, or overnights)

As a registered nurse at Heritage Christian Services, you'll work under the guidance of the Nursing Support Specialist to offer high-quality, person-centered care to people with intellectual and developmental disabilities. You'll work alongside a dedicated nursing team to provide clinical oversight to frontline staff members, including direct support professionals and licensed practical nurses, and build meaningful relationships with the people you support and their families. 


  • Conducts ongoing health assessments and supports the health needs of people in certified residential, day habilitation and respite settings. 
  • Responds to health concerns and medical emergencies with clinical guidance and support. 
  • Partners with physicians, specialists, pharmacies and medical liaisons to coordinate medical care and contribute to person-centered care planning 
  • Collaborates with medical liaisons to coordinate timely scheduling of appointments, hospitalizations and related services and ensures resulting care plans and medical recommendations are followed.  
  • Develops, implements and maintains individualized Plans of Nursing Service to coordinate nursing care and define responsibilities for direct support professionals. 
  • Delegates nursing tasks to direct support professionals in accordance with the New York State Nurse Practice Act and monitors and evaluates their competency in health-related supports and delegated tasks. 
  • Oversees medication management processes, including review of medication administration records and reconciliation. Addresses medication errors through investigation and follow-up.  
  • Participates in incident management, including clinical review of reportable incidents. 
  • Ensures all actions are compliant with OPWDD regulations, agency policies and administrative memoranda. 
  • Supports agency survey preparation, including participation in on-site surveys and remediation of identified deficiencies. 

  • Professional license and current registration to practice as a registered nurse in New York state as issued by the New York State Education Department. 
  • Background supporting people with intellectual and developmental disabilities and prior long-term care experience are preferred. 
  • Must demonstrate the ability and temperament necessary to work with and provide care to people with disabilities.
