The Source is seeking a compassionate, organized, and mission-drivenCase Manager to walk alongside women and families by providing care coordination, support, and access to life-changing resources. What You'll Do

As a Case Manager, you will:

  • Conduct client needs assessments and develop individualized Care Plans
  • Connect clients with community resources and government assistance programs
  • Provide ongoing encouragement, coaching, and care coordination
  • Conduct follow-up calls, home and hospital visits
  • Maintain accurate client documentation using Salesforce
  • Lead and support client groups while collaborating with clinic staff to ensure excellent client care

Required Qualifications

  • Committed Christian with a personal relationship with Jesus Christ
  • Passion for serving women, children, and families while upholding the sanctity of human life
  • Bachelor's degree in a related social services field, Registered Nurse (RN), or Certified Community Health Worker
  • Strong organizational, communication, and computer skills
  • Ability to manage confidential information with professionalism

Preferred Qualifications

  • Case management or care coordination experience
  • Experience in coaching, mentoring, or motivational interviewing
  • Master's Degree in Social Work (Texas Licensed)
