Job description

Overview
We are in search of a professional, compassionate, and dynamic Full-Time Licensed Practical Nurse (LPN) to join our Fellowship Square team. The candidate should be devoted to enhancing the lives of our Assisted Living residents, dedicated to addressing their daily needs, and committed to maintaining their dignity and quality of life.
The purpose of the Licensed Practical Nurse (LPN) position is to provide wellness services to the residents in the Oasis Assisted Living facility in accordance with the current applicable Federal, State, and local standards, guidelines, and regulations and our established policies and procedures.
Responsibilities
Every effort has been made to make this job description as complete as possible.
However, it in no way states or implies these are the only duties that will be required. Other related duties necessary to meet the needs of the residents may be assigned by the AL Manager.
Experience working in an Assisted Living community is preferred, mut opened to others depending on experience.
Prepare and update as required all resident service plans.
Perform assessments on prospective residents to determine the level of care for assisted living services.
Oversee the medication delivery plan for each resident including self-administration and medication monitoring, medication ordering, physician's orders and recapping MAR's to physician's orders.
Perform a daily narcotic count and maintain documentation of all narcotics provided to residents.
Schedule weekly showers and review caregiver documentation of ADL's.
Maintain resident injections such as tuberculosis skin testing; pneumonia vaccinations and B-12.
Write and coordinate policies and procedures for the memory care section of the Assisted Living facility.
Review and revise policies and procedures that govern the operation of nursing and caregiving services provided in the assisted living facility.
Assist the department directors in the development and use of departmental policies and procedures and establish a rapport in and between departments so each can see the importance of team work.
Qualifications
  • Must be a graduate of an approved school for Licensed Practical Nursing or Registered Nursing.
  • Must hold a current, valid LPN license in the State of Arizona.
  • Must pass a pre-employment drug test and background check
  • CPR/First Aid card, fingerprint clearance card, and current TB test required.
  • Understand DHS rules.
  • Previous experience in a geriatric setting preferred.
  • Must be flexible and have a desire to communicate and work with seniors.
  • Must be motivated, self-starter, with a passion for serving seniors.
  • Experience in an assisted living or long-term care environment preferred.
  • Schedule Availability: Part-time, flexible position on weekends with options to work additional hours.
