Job description

Job Purpose:

Under the general supervision of the Public Health Nurse/Community Health Manager, the Lead Community Health Representative (CHR) promotes the health and well-being of individuals, families, and the community through health education, outreach, advocacy, prevention, care coordination, and resource navigation. Serving as a liaison between patients, families, healthcare providers, and community partners, the Lead CHR supports access to culturally appropriate services, helps individuals understand and manage their health needs, and coordinates care across medical, dental, behavioral health, and support programs. Lead CHR also provides leadership within the Community Health Department by mentoring and supporting CHR staff, promoting quality service delivery, continuity of care, and positive health outcomes.

Qualifications:

  • A minimum of an associate's degree (or equivalent college credits) in Community Health and Human Services or related field.
  • Certified Medical Assistant. (CMA).
  • Certificate from CHR/CHW core competency training.
  • Minimum of three (3) year’s experience as a CHR/CHW.
  • Prior experience with providing community assistance in tribal specific programs and organizing health/wellness grants community events preferred.
  • Maintain patient confidentiality. Ability to work well within a multidisciplinary team.
  • Possess current CPR and First Aid certification or be willing to obtain within three (3) months of hire.
  • Must maintain a valid, unrestricted California Driver License and ability to be insured.
  • Must pass a pre-employment drug screen, physical and background investigation
  • Ability to work non-traditional hours including some weekends to assist supervisors to organize community health outreach events.
  • Must provide a current DMV printout.
  • Performs other duties as assigned.

Special Qualifications: All employees must be sensitive to the needs of the Native American community, demonstrate a deep understanding and respect for the diverse cultural heritage, traditions, and historical context of the Native American community.
