This position is fully onsite, as you will be visiting clients in their residences across San Jose. • Design and implement individualized physical therapy interventions to aid in pain relief, enhance mobility, and prevent disability based on comprehensive assessments and physician prescriptions.
• Supervise and mentor physical therapy assistants, ensuring that interventions align with established care plans.
• Conduct evaluations utilizing diagnostic tests to assess muscle, nerve, joint function, and overall physical capabilities, adjusting treatment as required.
• Assess clients' home environments to identify improvement opportunities that promote rehabilitation and increased independence.
• Deliver physical therapy services directly at patients' homes, ensuring a compassionate and empowering experience.
• Implement a range of treatment modalities, including manual therapy techniques, therapeutic exercises, physical agents, and educative support.
• Collaborate effectively in interdisciplinary team meetings to discuss treatment plans and patient progress, and share insights that improve patient outcomes.
• Maintain meticulous clinical records encompassing initial evaluations, care plans, and ongoing progress notes, ensuring compliance with relevant regulations.
• Educate clients and their families about home therapy techniques, the proper use of equipment, and health maintenance strategies.
• Participate actively in professional development and training initiatives to enhance your clinical skills and maintain up-to-date knowledge of best practices.

• A Doctor of Physical Therapy (DPT) degree from an accredited program.
• Minimum of one year of experience as a Physical Therapist, particularly in acute care or home healthcare environments.
• Current and valid state licensure for physical therapy, along with CPR certification.
• Valid driver’s license and automobile insurance to conduct home visits.
• Strong commitment to delivering high-quality clinical care with integrity and empathy.
• Ability to engage effectively with patients, families, and interdisciplinary teams, demonstrating strong interpersonal skills.
• Proficient knowledge of federal and state healthcare regulations, including those pertaining to home healthcare settings.

• Experience with electronic health records (EHR) systems to aid in accurate documentation and reporting.
• Background in developing and implementing patient care plans demonstrating measurable outcomes.
• Experience in community resource navigation that can assist patients in accessing additional support services.

Flex Care Chiropractic Center offers a competitive salary ranging from USD 90,000 to USD 110,000 annually, commensurate with experience and qualifications.
