Job description

CARE COORDINATOR

(OhioRISE)

Lighthouse Youth & Family Services is growing with our OhioRISE program!  We are hiring Care Coordinators.  We are looking for professionals that understand High-Fidelity Wrap Around practice while providing care coordination services to identified youth that will provide specific, measurable, and individualized services to each person served.

Competitive pay based on licensure and educational experience.

Starting hourly pay $21.00 and up.

Essential Functions:

  • Cultivate flexible, family-focused, community-based responsive services based on the High Fidelity, Wrap Around model of care coordination.
  • Applies High Fidelity Wraparound principals, including coordinating and leading team meetings as needed.
  • Coordinate and supervise implementation of the Plan of Care, including a Transition Plan and Crisis Plan, through service delivery with providers and community resources; update plan as necessary.
  • Ensure family support and stabilization during crises.
  • Provide and document the initial and ongoing Life Domain Assessment.
  • Maintain all service documentation requirements, evaluation outcome requirements and data.
  • Provide services in a timely manner and in accordance with Plan of Care and/or Crisis Plan.
  • Utilize and monitor Flexible Funding and service coordination.
  • Obtain weekly reports from subcontracted providers.
  • Participate in after hour on-call response.

Qualifications:

  • High School Degree with three years of relevant experience; OR Associate/Bachelor’s degree with 2 years of relevant experience; OR Master’s Degree with 1 year of relevant experience.
  • Experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice or behavioral health field providing community-based services to children and youth and their family or caregivers.
  • Experience in one or more the following areas of expertise:  Family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development
  • Excellent interpersonal and written communication skills.
  • Demonstrated computer proficiency.
