Job description

What You'll Be Doing
You will complete the Wheels of Change and field training to become a certified FCT and FCT-R Practitioner. The FCT-R Practitioner will provide in-home mental health and/or substance use treatment with the intent to keep families together, based on the Family Centered Treatment Model. You will support clients and families on their mental health and/or recovery journey by collaborating with a team of practitioners and Peer Recovery Specialists to promote recovery, stability, and the ability to maintain the family within the home.
Hybrid Role, 20% Remote Work.
Your Main Priorities
While additional responsibilities are likely to arise on the job, you will primarily:
โ€ข Work with clients and their family members and caregivers to identify opportunities and goals, assess functioning, develop a treatment plan, and assist in achieving desired outcomes.
โ€ข Build and maintain positive relationships with clients, family members and caregivers, and identify referring or collateral professionals and agencies.
โ€ข Collaborate with collateral contacts as needed.
โ€ข Provide two 2-hour treatment sessions per week with each client/family assigned.
โ€ข Flexible in providing services according to the client's needs.
โ€ข Maintains client records accurately, completes documentation requirements timely.
Who You Are
Requirements, Skills & Abilities:
โ€ข Bachelor's degree in social work of related field required; Master of Social Work degree preferred.
โ€ข Minimum 1 year experience in a similar role required; experience with trauma-informed and substance use treatment a plus.
โ€ข On call as needed.
โ€ข State wide travel is required.
โ€ข Strong organizational, critical thinking, and time-management skills.
โ€ข Basic computer skills required; knowledge of Microsoft Office 365.
โ€ข Ability to communicate and effectively interact with a diverse population.
โ€ข Bilingual in Spanish preferred.
โ€ข Collaboration and teamwork.
โ€ข Must authorize Child & Family to complete a motor vehicle records check.
โ€ข Must have valid driver's license, auto insurance and car registration.
โ€ข The ideal candidate will be required to meet all background checks and medical clearances required for the position.
Physical Requirements
Direct Service Staff:
This position requires time sitting, standing, walking, driving, carrying and lifting up to 25lbs. This role also requires operating desktop, laptop and/or cell phone, and communicating with colleagues.
