Job description

) We are hiring an Aftercare Coordinator to join our dynamic care team at Marlborough Hills Rehabilitation & Health Care Center!
8a-4p schedule with weekly pay. 32 hours a week. $23 to $25 an hour.
The Aftercare Coordinator provides planning for residents with substance use disorders in preparation for their discharge.
Experience & Education:
  • Preferred associate's degree in human services and or other relevant area of studies in this field.
  • Experience in aftercare coordinating and community support services preferred.

Duties & Responsibilities:
  • Assess resident needs, situations, strengths, and support networks to determine their aftercare goals.
  • Develop aftercare plans to improve the residents' well-being.
  • Help residents work with government agencies to apply for and receive benefits.
  • Develop and implement aftercare plans which are measurable and appropriate for the identified level of care.
  • Check-in with discharged residents.

Specific Requirements:
  • Must be able to read, write, speak, and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such actions.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
  • Must have patience, tact, cheerful disposition, and enthusiast, as well as being willing to handle residents based on whatever maturity level in which they are currently functioning.
