Clinical Case Manager (CHARTER Program)

The Company :

Client Dual-Diagnosis Enhanced Behavioral Health Treatment And Housing Provider And Recognized Leader Salt Lake Metro Area. They Specialize Delivering Evidence-Based Care Individuals With Chronic Behavioral Health Conditions, Including Histories Homelessness, Severe Mental Illness, Substance Disorders, Criminal Justice Involvement, Complex Medical Needs.

The Position :

Clinical Case Manager Provides Full Spectrum Clinical Case Management Services Within Scope Licensure. This Role Supports Individuals Experiencing Severe Mental Illness Co-Occurring Disorders Through Therapy, Service Planning, Coordinated Care.

Compensation & Benefits :

Responsibilities :

  • Monday - Thursday or Tuesday - Friday
  • Provide Therapy Case Management Services Individuals With Severe Mental Illness Co-Occurring Disorders
  • Conduct Resident Assessments Develop Individualized Service Plans
  • Facilitate Psychotherapy Groups Life-Skills Classes
  • Utilize Crisis Intervention, Motivational Interviewing, De-Escalation Techniques
  • Collaborate With Interdisciplinary Teams Including Counselors, Peer Support Staff, Medical Personnel, External Agencies
  • Coordinate Services Including Housing, Healthcare, Employment, Education Resources
  • Maintain Accurate Timely Documentation Using Electronic Health Records (EHR)
  • Engage Residents Using Trauma-Informed Care Evidence-Based Approaches Such ROEM Model
  • Assist With Eligibility Screenings Maintain Communication With Waitlisted Participants
  • Ensure Compliance HIPAA Regulations Through Proper Documentation Releases
  • Support Program Operations Attend Trainings Staff Meetings

Requirements :

  • Master’s Degree Social Work, Mental Health Counseling, Marriage Family Therapy
  • Active Utah License (CMHC, LCSW, LMFT)
  • Prior Case Management Experience Required
  • Experience Working With Chronically Mental Populations Required
  • Experience Substance Disorder Treatment Preferred
