Job description

Under the direction of the Program Director, the Chart Analyst will support all aspects of ensuring that program staff are adhering to service and documentation standards consistent with excellent client care and in compliance with county, state, and federal contractual regulatory guidelines.


Position Qualifications

  • Master’s degree in Social Work, Marriage and Family Counseling, Psychology.
  • Registered with the State of California Board of Behavioral Science Examiners for the area of specialty.
  • Must be able to pass California State Department of Justice criminal background investigation, Live Scan and/or fingerprinting.
  • Must have extensive quality improvement, DMH documentation and behavioral health chart review experience.
  • Must be detail oriented and possess effective verbal and written communication and teamwork skills with an ability to manage and support quality improvement efforts.
  • Ability to troubleshoot issues and partner with all levels of employees to identify solutions.
  • Know and comply and agency policies and procedures, HIPAA, LAC-DMH, Medi-Cal policies and documentation guidelines and other state/federal regulations related to quality assurance and management.
  • Basic computer skills and literacy are required to use the agency electronic health record. Staff must be able to logon, type, use a mouse and navigate through various screens or windows to complete daily chart documentation.
  • Demonstrate strong clinical documentation.
  • Must have valid California Driver's license and the availability of a car with adequate insurance.


Primary Duties:

  • Utilize and/or develop standardized quality assurance tools for the systematic review of services to meet contractual guidelines.
  • Monitor clinical documentation, capture data and measure compliance with all regulatory requirements using chart review tools.
  • Builds relationships with clinical staff to better serve as an onsite expert on quality and compliance issues.
  • Review 85 charts per month (including new cases enrolled, cases discharged, and random selection of active cases to meet monthly quota)utilizing the Case Record Review tool to ensure documentation meets SCHARP standard.
  • Communicate with management and clinical staff regarding documentation that is out of compliance and follow up to ensure items are corrected and re-submitted.
  • May assist with organizing and facilitating agency trainings.
  • Performs a wide-range of professional work in multiple administration functions within an organizational unit analysis and project management.
  • Assists with developing and conducting special studies and research initiatives by designing survey instruments, gathering data and information from department staff, developing data analysis tools, conducting statistical analyses, initiating literature reviews, etc.
  • Attend and participate in routinely scheduled program, department and agency staff meetings.
  • Assist with preparation of County and State audits, as well as prepares reports in responses to audits and plans of corrections.
  • Provide monthly report to Program Director of chart review including average scores, areas of deficiency, and areas further training may be needed.
  • Other duties as assigned.
