We are seeking a motivated and patient-focused Care Coordinator to support patients enrolled in Medicare's Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs.
This is not a traditional back-office Medical Assistant role. Instead, you will serve as a key point of contact for high-risk patients with multiple chronic conditions - helping them stay engaged, compliant, and on track with their care plans through structured outreach and monitoring. This on-site role offers a stable Monday through Friday schedule with no evening or weekend commitments.
Your Contribution:
  • Make 50+ outbound patient calls per day using our phone system
  • Utilize multiple client EHR systems for documentation
  • Conduct monthly check-in calls for RPM and CCM patients
  • Review and monitor synced vital signs from RPM devices
  • Order RPM devices and educate patients on proper usage
  • Conduct structured health assessments
  • Follow personalized care plans tailored to each patient
  • Provide education on nutrition, exercise, medication adherence, and lifestyle choices
  • Track and document patient progress accurately and consistently

Your Qualification:
  • Active National Certified Medical Assistant Required (To expedite the hiring process, please upload NCMA/NRMA credentials)
  • Minimum 2 years of healthcare-related experience
  • Minimum 1 year in related Chronic Care Management services
  • Strong communication and interpersonal skills
  • Comfortable with high-volume outbound calling
  • Strong documentation and EHR proficiency
  • Willingness to work on-site during regular business hours, Monday-Friday
