Job description

Job Overview and Qualifications

In this role, you will be responsible for delivering direct patient care in alignment with physician directives, adhering to established standards of practice, and following organizational policies and procedures. You will implement the care plan, administer treatments, and collaborate with team members to achieve favorable patient outcomes.

Your responsibilities will include:

  • Implementing the care plan, administering treatments, and collaborating with team members to achieve positive patient outcomes.
  • Delivering direct patient care as outlined in the State Nurse Practice Act, including specialized responsibilities related to end-of-life care.
  • Participating in the coordination of hospice services and effectively communicating identified needs to the interdisciplinary team, including Hospice Aide, Director of Clinical Services – Hospice, and Chaplain.
  • Providing effective pain and symptom management.
Qualifications Required:
  • Basic Cardiac Life Support certification must be obtained within 30 days of starting employment.
  • Registered Nurse (RN) license.
  • Valid Driver's License (DL).
  • Associate or Bachelor's Degree in Nursing.

Minimum of 1 year of relevant experience is required.
