ROLE OVERVIEW and PURPOSE
The Telehealth Registered Nurse will work to support the mission and vision of Neighborhood Healthcare by providing excellent patient-centered care and services within a dynamic team environment. This role will triage patients by evaluating and assessing their responses to determine care options. Additionally, this role will refer patients to a provider or recommend other treatment plans.
RESPONSIBILITIES
  • Answers patient calls on a wide range of healthcare topics during normal clinic hours
  • Assesses patients understanding of their condition and identify potential care and/or provider gaps
  • Evaluates, assesses, and triages immediate health concerns and documents outcomes; including determining if the concern requires emergency care or an appointment to see a provider
  • Searches a database of conditions and treatments for patients

Clinical Administration:
  • Provides patient advice and education on handling their situation
  • Provides ongoing training and support to Call Center Representatives (CCR) and others to assure effective and appropriate patient services through the Call Center

Patient Education:
  • Serves as a clinical resource to the Call Center and across the continuum of care to influence organizational standards related to patient safety and care delivery
  • Serves as a resource for patients by providing education and guidance to support positive health outcomes
  • Initiate patient education plan according to individualized needs, including patient and family instructions
  • Educates and instructs patients regarding lab tests, results, medication functions, and expectations to gain cooperation and allay apprehension

Customer Relations:
  • Maintains professional working relationships with all levels of staff, clients, and the public
  • Cooperate in accomplishing department goals and objectives

EDUCATION/EXPERIENCE
  • Associate's degree in nursing required; Bachelor's degree in nursing preferred
  • CA RN license required
  • Current Basic Life Support (BLS) certification through an American Heart Association (AHA)-approved source is required upon hire and must be maintained as a condition of employment.
    • AHA-approved courses include an in-person, hands-on skills check with a certified instructor using a mannequin to demonstrate CPR and emergency response techniques.
    • Online-only BLS courses without a live skills check do not meet this requirement.
  • Three years' nursing experience; nursing telephone assessment and triage experience preferred
  • Valid CA driver's license or out of state driver's license for active military personnel/spouses required
  • Proof of auto insurance required
  • Bilingual (English/Spanish) preferred

  • Excellent verbal and written communication skills, including superior composition, typing and proofreading skills
  • Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
  • Knowledgeable about and experience with assessing and solving problems on the telephone
  • Ability to use Microsoft Office applications
  • Ability to successfully manage multiple tasks simultaneously
  • Excellent planning and organizational ability
  • Ability to work as part of a team as well as independently
  • Ability to work with highly confidential information in a professional and ethical manner

Pay range: $50.09 to $75.09 an hour, depending on experience and additional qualifications.

While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to the candidate's overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs. As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.
