Description
We are seeking a
RN-Nurse Educator
to join our team!
The qualified candidate will join the Envision Wellness interdisciplinary team and provide evidence-based diabetes self-management education and support (DSMES), nursing assessment, chronic disease education, care coordination, and population health services. Responsibilities include developing individualized care plans; providing diabetes education; supporting CGM, insulin pumps, and other diabetes technologies; coordinating care with providers and other healthcare professionals; conducting patient outreach and follow-up; and participating in community wellness, health screenings, quality improvement, and program development. Candidate must provide excellent patient care, communicate effectively, work independently and collaboratively, and have strong computer/EMR skills.
REQUIREMENTS
Required: Current Registered Nurse (RN) licensure in Missouri; minimum of two years of clinical nursing experience; excellent communication and patient education skills; ability to manage competing priorities in an ambulatory care environment; and ability to work independently while collaborating with an interdisciplinary team. Candidate must obtain and maintain Certified Diabetes Care and Education Specialist (CDCES) certification within the timeframe established by Northeast Missouri Health Council. Preferred: experience in primary care, endocrinology, population health, ambulatory care, chronic disease management, patient/group education, diabetes technology (CGM/insulin pumps), care coordination, or EMR systems.

REQUIREMENTS
Qualifications include candidate to preferably have a minimum of two years' experience in a healthcare setting, but not required. Certified Diabetes Care and Education Specialist (CDCES) certification is a plus, but can also be willing to obtain certification within the first 2 years of employment. Candidate must have a current Registered Nurse licensure (RN) in the state of Missouri
