Terros Health is hiring a CCBHC Coordinator at our 27th Ave location in Phoenix, AZ.

Location: 3864 N 27th Ave, Phoenix, AZ 85017

Schedule: Full-time

Monday - Friday, 8:00am - 5:00pm

Position Summary: The CCBHC Program Coordinator is responsible for supporting the implementation, coordination, and ongoing operations of the Certified Community Behavioral Health Clinic (CCBHC) program. This role ensures alignment with grant requirements, regulatory standards, and organizational goals. This position plays a key role in program oversight, data collection and validation, compliance monitoring, and quality improvement efforts. The CCBHC Program Coordinator partners with clinical, operational, and leadership teams to support program fidelity, reporting accuracy, and continuous improvement, while providing training and technical assistance to staff.

Duties Include:

  •  Coordinates daily CCBHC program activities in collaboration with clinical leadership, operations, and grant stakeholders
  • Supports implementation of grant objectives, timelines, and deliverables
  • Facilitates communication across multidisciplinary teams to ensure program alignment and execution
  • Conducts audits of clinical documentation and patient records to ensure compliance with CCBHC standards and grant requirements
  • Monitors adherence to program fidelity standards and identify areas of risk or improvement
  • Supports development and implementation of corrective action and process improvement plans
  • Coordinates collection, tracking, and validation of client-level data (baseline, reassessment, annual measures) in alignment with CCBHC reporting requirements
  • Prepares and distributes regular reports and dashboards summarizing audit findings, trends, and key performance indicators
  • Partners with program evaluators and leadership to monitor progress toward grant goals

  • A minimum of High School diploma or GED; bachelor’s degree preferred
  • Minimum 3 years of experience in behavioral health, healthcare operations, or program coordination
  • Experience with grants, compliance, or quality improvement preferred
  • Strong analytical and organizational skills with attention to detail
  • Ability to interpret data, identify trends, and support quality improvement efforts
  • Effective communication skills with a collaborative, non-judgmental approach
  • Proficiency in Electronic Health Records (NextGen preferred), Microsoft Office and data tracking and reporting tools
  • Must have a valid Arizona driver’s license, be 21 years of age with a minimum of 3 years driving experience, and meet requirements of Terros Health’s driving policy
  • Must successfully pass a TB screening and criminal background check
  • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
