We are seeking a strong MDS Nurse and leader in the sub-acute and long term care market! The candidate we are seeking is an experienced, energetic, and compassionate RN or LVN to serve as our MDS Coordinator. We see a diverse mix of cognitive, orthopedic, and neurological conditions with short term rehab and long term care residents and there is excellent potential to build amazing programs with our excellent clinical team, including Montessori Memory Care, Stroke Rehabilitation, and many more. Our veteran team of clinicians is a dedicated team who are fun, upbeat, and passionate about providing outstanding care for our patients and residents.
We seek a nurse professional with strong people skills to work with our patients to establish short term and long term care planning, ensure appropriate reimbursement by Medicare, Medicaid, and other insurance plans, follow up to make sure patient care is appropriate and patient goals are being met, and work with our facility team to create continous improvement of the services we provide. Must have Texas MDS experience!
