Job description

Archstone Behavioral Health is seeking a compassionate, organized, and dependable Case Manager to join our team. The Case Manager will support clients throughout their treatment experience by helping coordinate services, addressing barriers to successful treatment and discharge, and connecting clients with appropriate community resources.

The ideal candidate will have strong communication and organizational skills, the ability to maintain professional boundaries, and a genuine commitment to helping individuals achieve stability and long-term recovery.

Essential Duties and Responsibilities

  • Provide case management services to clients receiving behavioral health and/or substance use treatment.

  • Complete case management assessments and identify client needs, strengths, and barriers.

  • Assist clients with obtaining appropriate community resources and support services.

  • Coordinate referrals for housing, transportation, employment, education, benefits, medical care, and other community services as appropriate.

  • Assist with discharge planning and ensure appropriate continuity of care.

  • Coordinate with therapists, medical staff, nursing, utilization review, admissions, and other members of the treatment team.

  • Participate in treatment team meetings and provide updates regarding client progress and identified needs.

  • Maintain accurate, timely, and complete documentation in the EMR.

  • Maintaining appropriate professional boundaries.

  • Communicate with outside providers, agencies, family members, and other approved supports as appropriate and in accordance with confidentiality requirements.

  • Follow all HIPAA, DCF, facility, and applicable regulatory requirements.

  • Maintain confidentiality of all client information.

  • Identify and report safety concerns or changes in client status to the appropriate supervisor or clinical staff.

  • Attend required staff meetings, trainings, and continuing education.

  • Perform other duties as assigned by the supervisor or Clinical Director.

Qualifications

  • Relevant experience in behavioral health, substance use treatment, case management, social services, or a related setting preferred.

  • Strong written and verbal communication skills.

  • Excellent organizational and time-management abilities.

  • Ability to work effectively as part of a multidisciplinary treatment team.

  • Ability to maintain professional boundaries and confidentiality.

  • Knowledge of community resources and behavioral health services preferred.

  • Must demonstrate professionalism, reliability, sound judgment, and compassion.

  • Must be able to successfully complete required background screening and other pre-employment requirements.

Preferred Experience

  • Experience working with individuals experiencing substance use disorders and/or mental health concerns.
