Job description

Under the supervision of the Program Manager, the incumbent will serve as a Case Manager to support individuals in recovery by providing case management, aftercare coordination, and linkage to Tribal and community-based recovery services. The incumbent will serve as a Case Manager to support individuals in recovery from substance use disorder, specifically opioid use disorder. The incumbent will serve as an advocate, care coordinator, resource broker, and liaison to help individuals achieve and sustain long-term recovery.
