We are seeking a highly organized and compassionate Nurse Case Manager to join our patient-centered care coordination team. You will review clinical information, monitor progress toward goals, advocate for patient needs, and work closely with physicians, therapists, social workers, and insurance providers to eliminate barriers to care.
