$2,000 Sign on Bonus

Blended Case Manager (BCM), will promote resiliency and recovery forchildren, adolescents, adults and their families by accessing, coordination, mentoring, and advocating for community resources and services.

Bachelor's Degree in Human Services or related field. Must work 40 hours per week, have flexible schedule, and on-call is required. Blended Case Management experience preferred.
