Job description

We are seeking a compassionate and dedicated Case Manager/QMHP to join our team. The ideal candidate will have a strong background in case management and a passion for helping individuals in the behavioral health field.
Qualified candidates MUST have a minimum of a Bachelor's a degree in: Psychology, Social Work, Sociology, Psychology, or Counseling.
Duties/ Responsibilities

• Conduct thorough assessments of clients’ needs and circumstances.
• Develop and implement individualized case management plans.
• Provide behavioral support to clients.
• Collect data to monitor client progress and outcomes.
• Coordinate care with other professionals, including healthcare providers and educators.
• Advocate for clients’ needs within the community and healthcare systems.
• Maintain accurate documentation of client interactions and case progress.
Minimum Skills
/Requirements 
  • Bachelors Degree or higher in psychology, social work, sociology, educational psychology, or counseling
  • Good listening, verbal, and written communication skills
  • Good work ethic to complete tasks independently
  • Functional knowledge of computer programs (Word, Excel, Web based programs)
  • Must meet state requirements for QMHP status (CANS and ANSA Cert)
  • Experience working with youth or families in a behavioral health setting
  • Good driving record with at least 3 years of driving history
  • Reliable vehicle with insurance
  • Bilingual in English and Spanish preferred but not required
